Look this survey questionnaire (http://www.zoomerang.com/recipient/survey-intro.zgi?p=WEB224F7T7UTM3) linked into the JOnAS page (http://jonas.objectweb.org/): Zoomerang (http://info.zoomerang.com/) is providing survey softwares and even hosting capabilities. So far, I dont think full OPS potential has been achieved.
